Error Message "ExecuteQNR has stopped working"

after filling in a questionnaire and assigning it to a letter, we get the following error message "ExecuteQNR has stopped working"...any thoughts? The letter still posts but it seems it should post directly from the previous screen.

Good day Dr. Singer,

I have called your office to find out what version of Imaging you are running but got your VM.  I will be reaching out again on Monday 5/16/16 and once I have confirmed the Imaging version, will have the latest version shipped.

Good day Dr. Singer,

What versions of Orthotrac and Imaging are you running?  There was a defect identified with version 11.5.0.67  of Ortho Imaging that is resolved in version 11.5.1.34.

Hi, how do we figure out what version of imaging we have. We just

started with Orthotrac two weeks ago and I assume we have the latest

version...maybe a bad assumption?

Good day Dr. Singer,

The Orthotrac and Imaging version numbers are in the bottom right corner of the main Orthotrac toolbar itself.  In the below screenshot I am running version 12.4.2.14  of OrthoTrac  (still in beta at this time) and 11.3.2.3 of Imaging. 

If you are on a version of Imaging prior to 11.5.1.34, please let me know and I will have the latest version shipped out ASAP.
