Hi Laura,
You can do this by following these steps...
1. From Main Menu of R4, go to Patients and load any patient
2. Click Medical tab
3. Click View Questions
4. Click New Question if creating new question, click on a question and then click Edit if editing a question.
5. Fill out Question Properties and make sure to move the question to the "questions included in questionnaire" list.
