R4+ includes the ability to complete an ACORN Assessment form when seeing NHS patients in Wales. If an assessment form is completed, it is stored on the patient's record and can be referred to at later visits allowing you to see any changes that may have occurred to a patient's oral health between assessments.
If an ACORN Assessment form has been completed within the last 12 months, the Clinical Users tab on the FP17 claim form will be automatically populated with the data from the most recent assessment form.
NOTE: You must manually select ACORN Assessment Carried Out on the Clinical Users tab if the assessment was completed as part of this course as it is not automatically selected.
