We use E-forms in our very large practice. Takes a bit to get them set up and to get your current forms into use but Carestream can help you with the process. We send the eforms out for all new patients when scheduling the appointment. Must get the data such as date of birth, cell phone and email address; then you send eforms out to be completed. Accept them back to import data when complete. We also use iPads to allow patients when checking in the complete forms such as medical alerts, hippa etc. Not everything is seamless but it works well for us. Medications do not transfer over and that is a problem but we just enter ourselves in the patient information medical alert screen. I hope this helps in making a decision to utilize these forms. ambrunst983, we were super excited about them when they first came out, however we have since turned them off. We too were using ipads to allow patients to check in using the eforms. However they were continually going up and down which was frustrating to our patients as well as our front office. Since we don't like our patients to be frustrated we went back to paper forms. They are also hard to use from home (so our patients tell us) as in, if you forget your username and password (which mostly only use 6 months), it's not easy at all to retrieve a new one. We would love to have them up and running again, but it seems that isn't in the works anytime soon. Hope this helps!
