we did have a similar issue and discovered that the operator switched sensors in the middle of it from a 2 to a 1. You either have to be prepared before hand for a second sensor, or save what you have before you add another sensor. Also is it always the same computer workstation doing this?
Disappearing act has never happened to us...yet. But, several times, we have taken xrays in the wrong chart (husband decided to go first before wife, etc) and had to move them later.
We are on the SD cloud. Are all of you? Or local SD users?
This has happened daily in our office as well, BUT only when we are taking a full mouth set of x-rays. Through a long drawn out process (over a year) we have come to the conclusion that it is for some reason the "size" of the mount. So... the assistants do not scan 4 of the #2 size films and then the full mouth goes in their mount and they just have to attach the 4 films to the patient when they are scanned. (We use a plate system). Good luck !!! this is a frustrating issue!!!
