Is there a way to default service facility box 32 on HCFA form to default?

Jump to solution

It default to blank and rejects under some insurances if not filled out.  Since 90% of what we do is in our office how to we get it to default to our service facility and then we just manually change it if doing the case at a hospital?

If these are paper claims then you can create a forms Designer rule to automatically default the service office and npi to boxes 32 and 32a respectively. To create a rule in WINOMSCS

  1. Select Utilities > Forms Designer. The Forms Designer Manager window is displayed.

  2. Click New. The Forms Designer wizard is displayed. The wizard screens enable you to perform the following tasks to customize a form - click Next

  3. Select an insurance form layout to customize by selecting the form number used by the Plan i.e  112 or 113
  4. Assign a layout to a plan by clicking the ellipses to search for the plan. Leaving this box blank means the rule will affect all plans using the form number selected - click Next

  5. Assign a customized insurance form to a specific provider and office location. Use the drop down to assign the rule to a specific or all Providers and to a specific or all Offices. - click Next
  6. Click Finish and a blank claim form to appear

  7. Right click on Box 32 and select Service office

  8. Right click on Box 32a and select either Provider, Office, or Practice NPI (if required). Office NPI has to be set in Tables > Practice > Offices. Practice NPI is under Tables > Practice > Practice Preferences > Insurance (look for NPI ID at the bottom)

  9. Click Ok to save rule

Once the rule is active it will need to be disabled so that the facility that is select during the posting process prints on the paper claim. To deactive go into Forms designer and uncheck the active box till teh claim has been printed then reactivate.

You can add your office as a Service Facility so that it can be attached during posting which can help if getting eClaim rejections.

I hope this helped
