Hey! Our surgeon(s) did a case in the hospital. We are billing through our office address, but I need to change the Service Facility Location Information in Box 32 on the CMS-1500 form to reflect the hospital's address. I have tried to change it and it will not update on the claim form. The process I followed was this:
I entered the charge and under the Service Information, I chose our doctor as the Provider, our office location for "Svc Office" and the hospital we used in the "Facility" dropdown.
It refuses to reflect this information on the claim form, and instead has our office address in both boxes. Am I missing something?
You might have a form designer setup for that plan or all plans setup for that box. Under Utilities > forms designer option, look for the plan or all plans and see what is being force to default in that box.
