Hello OMS people, I have some questions about Truforms. If you receive incomplete or unsigned forms are you having the patient sign and complete them in your office? Can you sign the forms once the Truform is converted? Are you printing the Truforms having the patient complete and sign the form and scanning them back in? Do you have paper forms for the patients that do not complete them before their appointment? If so are you scanning these paper forms and saving them in the EMR under Health History? How do your offices handle these types of situations?

We print them and have them in the chart for the patient to finish and then sign We then scan them when we are done.
We do the same thing. Print and have the guarantor review the paperwork and sign when they come into the office. We have found that many times the parent that brings the child to the appointment, is not the one who filled out the paperwork online. Our practice requires that the guarantor be the person who brings the patient to the appointment as they are the one speaking with the doctor and signing the consent forms. This also helps to avoid a lot of issues with divorced couples who like to put their exes on the form as responsible party.
