Issues with Paper Conversion Claims

It has been discovered that an error in processing mailed claims returned to Change Healthcare by the US Postal Service has caused some providers to receive paper claims in the mail from Change Healthcare which do not belong to them.  Should a patient contact you concerning such a mailing please ask that they either email scans of the claims/envelope or mail the claims and envelope to the address below.  If email is chosen please ask that the paper claims be destroyed after emailing them to Change Healthcare. This will be posted to the Carestream Community as well.
