Missing/Invalid patient *** and workspace default

Rejected claims: We seem to be having a lot of claims rejecting for patient *** missing.  The girls are all sure they are entering it.  Does anyone know of a glitch where it is entered and then disappears?  I don't know what else to do its happening at least 5 times a week if not more.  We are using version 9.2.064.  


Also in this version we keep having to safe workspace preference to front desk.  even after we save it it goes back to EMR the next day and we have to save it again.
