CS Remote Forms

Following on from our Webinars covering CS Remote Forms I am happy to share a link to register interest in this service.

This service provides the ability for practices to send medical questionnaires and COVID triage forms to patients for completion at home, prior to any practice visits.

I need guidance on how to manually send the remote access forms

Hi,

Once you have logged in to the website (www.surveyscreator.com)

  1. Click on Appointments to access a list of the currently booked appointments from your CS R4 system.
  2. Select the patients that you wish to send the forms to by ticking the box beside their name.
  3. Select the forms that you wish to send the relevant patients.
  4. Click Send Email.

All of the selected patients will receive an email with links to the selected forms.

As of Monday, there will also be some quick start guides for easy navigation.  We will post them here, we will also email them to all existing users.

Hi there - as with other practices, the remote forms were installed and some are being sent to patients before any training was given to us! Hopefully all will become clear after I have attended tomorrow's webinar. We have had several cases of patients completing and submitting forms but they haven't reached us! It's getting embarrassing to keep phoning these people back....

I also would like to know whether R4 has to remain open on the server in order for the forms to be sent?

Hi,

When a patient completes and submits a form through CS Remote Forms the form is automatically uploaded to their CS R4 patient record and will appear as received letters in their Comms tab, so it is always worth checking there first before contacting the patient.  If this is where you are looking and they are not appearing there then please let me know and I will pass on the information to Welltime for them to investigate further.

If the patient does not complete the forms then you will receive an email 30 minutes before their appointment to let you know that they have not been completed.  At the present time there is no way to extend this from 30 minutes.

Although your server needs to be left on, R4 does not need to be running in order for the emails to be sent out after 6:00pm.

Webinairs are fine, but I can't spend an hour during the day to watch them, can we either have printouts or access to a recorded webinair.

The webinars are not being recorded but there will be handouts available at the end of the week which will cover editing the existing templates and creating new templates

There have been a large number of changes made to CS Remote Forms since it was first introduced so the initial material is waiting to be updated.

If you can let me know what you are trying to do then I will see if I can help you.

CS forms is now setup and initial impressions are good however It was alll setup remotely & I have no informaion on how to add/ modify etc.

1.Responses are being received by the wrong E-mail address. No one has asked me how I would like this setup

  1. I have no idea how to administrate change/add forms etc
  2. Medical history form is of limited use as it is as simple tick yes that they are receiving treatment or taking drugs but no ability to enlighten us with free text as to what the treatment might be or what drugs it is they are actually taking, this makes the information I am receiving of limited use.

 

Hopefully someone will contact me on  how to streamline and improve things.

A PDF manual would be nice.

Same here. We had Remote Forms installed remotely on Thursday last week, but need guidance on how it works, and how it can be modified to our needs.

Lots of our patients are managing to fill the forms in & the automated arrival into the Comms Tab is very neat: We are adjusting our other patient Comms to make them aware of what is happening. To be aware of the E-mail address that they are coming from & that the DOB & post code need to be entered in the perfect format for the links to work. With more detail on the medical history and it will be perfect

I am currently working on some handouts on how to create new forms and modify the existing forms that can be sent out in CS Remote Forms.  In addition, we are planning on running a few free webinars on CS Remote Forms.

Im trying to create a survey form which I will use to send my Google review link.

This doesn't seem to be working as it comes back with the message 'www.google.com refused to connect'

is there a problem with using links?

Any sign of this yet, we supposedly had this installed last week, but have no idea how it has been set up and how it operates, would really appreciate some feedback ASAP. As we reopen tomorrow, we currently have no idea what patients will be receiving if anything.
