SMS - Send out NPS Survey via HCL SMS Service

Raising on behalf of Crouch Hall Dental Care. 

R4 Suggestion: 
Add a functionality to allow for an NPS Survey template to be sent to a patient upon completion of an appointment, along with response measurement for rating aspects of the service from 0-10. 

Responses to be able to be reported upon, building statistics for performers and other employees who interact with the patient at the practice.
