Changing the Signature Wording on Treatment Plan

Is there a way to change the verbiage where the signature box is on the treatment plan when printed?

Re: Changing the Signature Wording on Treatment Plan

Hey Karen, unfortunately the Treatment Plan form customization is limited. You can change the Treatment Plan form you are using by following these steps :

  1. On the Menu Bar, Click "System"
  2. Click "Form Selection"
  3. Change the "Treatment Plan" dropdown to the desired form

There are only a few forms to choose from, and may not have any changes to the Signature Box as you requested. One item you can edit, however, is the Comment Box. You can add or remove predefined comments by following these steps :

  1. On the Menu Bar, Click "System:
  2. Click "Change System Settings"
  3. Click "Treatment Plan Comments"

Any comments you create here can be added when printing your treatment plan.

At the moment, these are the only way to customize your Treatment Plan form.

Thanks Anthony,

We already knew how to do that, I would like to specifically be able to edit the signature box itself.

Re: Changing the Signature Wording on Treatment Plan

We would like to be able to change the wording on the treatment plan as some patients refuse to sign with the wording 'I agree to have the treatment carried out'. They think it ties them in to something they may change their mind about at a later date. They are really signing to say the treatment options & costs have been discussed with them & they have received a written copy of the treatment plan & estimate.

Re: Changing the Signature Wording on Treatment Plan

Which treatment plan form are you using? The one we're using doesn't have a signature box, and I can't figure out which form(s) do have a signature box.

Thanks.

Re: Changing the Signature Wording on Treatment Plan

There are only 2 forms that appear on the ipad for signature 1. patient medical history 2. patient treatment plan both of which were pre-installed by carestream via the R4 system we use. There is only 1 treatment plan which appears for signature we dont & never have had a choice.

Re: Changing the Signature Wording on Treatment Plan

Hello @Jobbie!

I see that you mentioned that you have R4. We have a specific space on The Exchange for R4. This discussion that you commented on is actually in the area for our SoftDent software product.

So the R4 team receives your question, you can go to PRACTICE MANAGEMENT at the top of The Exchange and select R4+ to post in their area, or you can use  this link to get there  https://community.carestreamdental.com/t5/R4/bd-p/r4plusdiscussion

They should be able to answer any question you may have! Have a great day!
