Invalid eClaim Rejections - November 2016

Some eClaims have come back with the following rejection message:


Reject reason: ERROR- Max use exceeded. Segment REF (Billing Provider Tax Identification), ERROR DATA- REF 0B 7588 ERROR- Max use exceeded. Segment REF (Billing Provider Tax Identification), ERROR DATA- REF 1B 217568797001

These claims were processed by the inMediata clearinghouse.  An error occurred in their system that generated these invalid rejections. 

An issue was discovered on 11/16/2016 with dental claims rejected in their front-end system. The inMediata clearinghouse is working on the problem. The error is affecting claims that report license number at the Billing Provider level only. Claims without license number or reporting license number only at Rendering Provider level are processing fine. They expect to have the problem fixed on 11/17/2016 . They will also reprocess all affected claims once the issue is resolved.

Please Note: There is no action required on behalf of the provider.

Carestream has received confirmation from the inMediata clearinghouse that the error in their front-end validation was corrected late yesterday, 11/17/2016. All affected claims have been reprocessed and are been sent out to the corresponding payers.

Once again, providers can simply ignore the error message and there is no action required on their part.
